Physician Information
To refer a patient, please fax an order and facesheet to 833-925-2447 for PT Wound Evaluation & Treatment, the patient will be scheduled promptly. An order is not required for treatment, but helpful. Wound Therapy will ensure the patient has a PCP or podiatrist to assist in signing orders for treatment. Feel free to reach out via phone, text or email with questions, 317-961-2121. The patient is welcome to call and schedule to prevent delay in treatment. Feel free to send order AND have patient call to schedule to prevent delays in treatment.
Wound Therapy works with the physician, podiatrist and/or nurse practitioner to ensure all needs are met. This includes obtaining DM shoes, compression garments, and dressings for home. Goals include teaching prevention of wound reoccurrence as needed. Collaboration with the patient, their family and healthcare providers allow a team approach leading to long term success.
